PathPoint Health and Ascension St. Vincent’s recently opened PathPoint Health – Jacksonville Southside, the joint venture’s first clinic in Northeast Florida. The clinic is PathPoint Health’s first location outside of Tennessee, where it operates three clinics in partnership with Ascension St. Thomas.

“Opening our doors in Jacksonville is a milestone we’ve been working toward since announcing this partnership,” said Grant Rutledge, founder and CEO, PathPoint Health. “Together with Ascension St. Vincent’s, we’re bringing coordinated, whole-person metabolic care to patients in Northeast Florida, working with referring physicians and care teams to deliver integrated care.”

“Metabolic conditions like diabetes and obesity can affect nearly every aspect of a person’s health, which is why patients can benefit from care that looks at the whole person,” said Cory Darling, president and CEO, Ascension St. Vincent's Southside. “Through our partnership with PathPoint Health, we are helping make it easier for people across Northeast Florida to access the resources they need to improve their health and manage these conditions over the long term.”

PathPoint Health – Jacksonville Southside pairs medical weight loss, including GLP-1 prescription and management, with lifestyle support, including nutrition and exercise planning and behavioral health support, through a combination of in-person and virtual visits.

The clinical team is led by Salma Makhoul Ahwach, M.D., who joins PathPoint Health as market medical director for North Florida. Ahwach, who is board certified in endocrinology with a clinical focus on the metabolic drivers behind weight, has practiced as an endocrinologist in Jacksonville since 2020 and will focus on weight-related metabolic health improvement as part of her move to PathPoint Health. She is fluent in Arabic and French, with working proficiency in Spanish.

“Every patient who comes to us gets a care plan built around their health history and their daily life, and a team that stays with them for the long term,” said Ahwach. “I’m proud of our Jacksonville team that is ready to partner with patients and the physicians who care for them.”

For patients pursuing weight loss surgery, PathPoint Health partners closely with Ascension St. Vincent’s bariatric surgery program to coordinate perioperative care. Patients receive support preparing for surgery and continued follow-up afterward, so their care stays connected before, during and after their procedure long into recovery.

PathPoint Health – Jacksonville Southside is located at 4205 Belfort Road, Suite 4030, Jacksonville, and serves patients across the Greater Jacksonville Area, including St. Johns County and Clay County. The clinic is open Monday through Friday, 8 a.m. to 4 p.m.

New patients can self-schedule an appointment online at pathpointhealth.com, with no phone call needed. For questions, call 904-515-6621.
