Dr. Sarah Adams recently opened the new Nocatee location of her practice, Palm Valley Family Medicine. She spoke about this exciting new chapter, her passion for helping people be their best selves and what makes Palm Valley Family Medicine unique.
Tell us a bit about your practice and the services you offer.
Palm Valley Family Medicine is a longevity concierge-style, direct primary-care practice designed to bring medicine back to what it should be — personal, accessible and relationship-driven. I care for both pediatric and adult patients, offering everything from preventive care and same-day visits to chronic disease management, hormone optimization and longevity-focused medicine.
My patients have direct access to me through text, phone and telehealth, along with longer, unhurried appointments. I also offer curated services like advanced lab testing, peptides, skin health solutions and IV therapy — creating a truly comprehensive, modern approach to wellness under one roof.
Tell us about your background, especially as it applies to your career.
I’m a board-certified family medicine physician and a diplomate of the American Board of Family Medicine. My career has always been rooted in education, leadership and patient advocacy. I’ve served in academic medicine, held leadership roles in residency training and currently serve as chair of the Direct Primary Care Member Interest Group for the American Academy of Family Physicians.
What do you like best about what you do?
Without a doubt, it’s the relationships. I get to truly know my patients — their families, their goals, their struggles — and be a consistent presence in their lives. I also have the privilege of helping people become their best selves.
In traditional medicine, so much of that connection is lost to time constraints and system pressures. In my practice, I have the ability to slow down, listen and partner with my patients in a meaningful way. That’s where real healing happens, and it’s incredibly fulfilling.
What makes PV Family Medicine unique?
What sets us apart is the experience. This isn’t just health care — it’s personalized, elevated care that feels completely different from the traditional model.
Patients have direct access to their physician, minimal wait times and a level of attention that’s rare in today’s health care system. We also blend traditional family medicine with forward-thinking services like hormone optimization, longevity care and proactive pediatric care, so patients don’t have to go to multiple places to feel their best.
It’s a boutique, longevity- and relationship-based model designed for people who want more from their health care.
Have you recently started accepting patients in Nocatee?
Yes! I recently opened my Nocatee location, and it’s been such an exciting step for the practice. The community here aligns perfectly with what we offer — families and individuals who value accessibility, personalized care and a proactive approach to their health.
We are currently accepting new patients, and it’s been incredibly rewarding to grow within this vibrant, health-conscious community.
How do you like to spend your free time?
When I’m not working, I love spending time with my family; it’s my reset. My grandmother, Odie McClintock, is my favorite person, she is a retired schoolteacher who deeply shaped me as a person. When I can I try to get her pearls of wisdom.
I truly believe you can’t pour into others without taking care of yourself first, and I try to model that both personally and professionally.
What would you like our readers to know about PV Family Medicine?
I want people to know that health care can feel different in the best way. It can feel personal, accessible and even empowering. This is a practice rooted in longevity and proactive about health. As a part of my practice, you feel a part of the family.
Palm Valley Family Medicine was built to give patients a better experience, one where they feel heard, cared for and supported in every stage of life.
