Dr. Sarah Adams recently opened the new Nocatee location of her practice, Palm Valley Family Medicine.
Tell us a bit about Palm Valley Family Medicine.
Palm Valley Family Medicine is a concierge-style family medicine and direct primary-care practice focused on creating a more personal, accessible and relationship-centered health care experience. I care for patients of all ages and focus heavily on prevention, longevity, hormone optimization, women’s health, pediatrics and overall wellness. My goal has always been to bring medicine back to what it should be — a partnership between doctor and patient where people feel heard, valued and truly cared for.
We offer longer appointments, direct access to me, telehealth, in-person care and a personalized approach that allows us to focus on root causes instead of rushed visits. We’ve also recently expanded our pediatric home visit option for families in Nocatee, Ponte Vedra, Deerwood and SilverLeaf, making care even more convenient for busy parents and sick children who are more comfortable being seen at home.
As the practice continues to grow in the community, I’m excited to continue expanding services that help patients live healthier, fuller lives.
What first sparked your interest in medicine?
I’ve always been drawn to helping people, but my passion for medicine really grew from wanting to make a meaningful impact during some of the most vulnerable moments in people’s lives.
What can new patients expect from Palm Valley Family Medicine?
New patients can expect a very different health care experience from the traditional model. We prioritize accessibility, communication and personalized care. Patients have direct access to their physician, longer visits and the ability to truly discuss their health goals without feeling rushed.
My patients and families become members of my family. I’ve had patients tell me that visiting my office is like coming over to a family member’s house. With our longevity approach and our geriatrics program geared towards longevity, older people can feel like they have a safe space to talk about how to keep their bodies healthier longer and how to stay forever young!
What is a typical day like for you?
No two days are exactly the same, which is one of the things I love most about family medicine. My mornings usually start early with movement, coffee and time with my children before diving into patient care. During the day, I’m seeing patients, answering messages, reviewing labs, collaborating with my team and working on growing the practice.
Because I run a concierge-style practice, I’m able to spend more meaningful time with patients and really focus on individualized care. Outside of clinic hours, I’m often working on community events, wellness education, social media content or brainstorming new ways to improve the patient experience. It’s busy, but incredibly fulfilling. It is very, very important to me to be an active member of community and give back in any way that I can!
Are you from this area originally, or did you settle here from somewhere else?
I settled here from Columbus, Georgia, and it quickly became home. I was drawn to the strong sense of community, the family-friendly atmosphere and the lifestyle that Ponte Vedra and Nocatee offer.
As both a physician and a mother, I loved the idea of building a life and practice in a community that values health, connection and family. The support from this community has been incredible, and I feel very grateful to serve the people who live here.
What do you like best about living here?
What I love most is the sense of connection and community. People here genuinely support one another, and there’s such an emphasis on family, wellness and enjoying life. I also love being close to the beach, spending time outdoors with my children and having access to so many amazing local businesses and community events.
It’s a beautiful place to raise a family and build a practice, and I truly feel lucky to be part of it.
What’s next for you and Palm Valley Family Medicine?
The future is very exciting. We are continuing to grow and expand services while staying true to our mission of personalized, patient-centered care. My vision is to continue building a modern family medicine practice that blends excellent medical care with wellness, longevity and accessibility.
I’m especially excited about expanding community education initiatives, women’s health and hormone optimization services and continuing to create a health care experience where patients feel genuinely cared for.
More than anything, I hope Palm Valley Family Medicine continues to be a trusted place where families can feel supported through every stage of life.
We have now opened an additional office in Nocatee, and this offers pediatric home visits in Nocatee and in Ponte Vedra and even all the way into the 210 neighborhoods by a board-certified pediatrician! We have also expanded our geriatrics and longevity program, and I’ve also collaborated with a male physician for patients desiring a male provider.
